Choptank Community Health System
Pharmacy Assistance Programs
If you are unable to afford prescription costs, CCHS may be
able to help. CCHS provides access
to medications for patients, primarily through two different programs. The pharmacy
assistance programs are based on income and existing
pharmacy coverage with established insurance programs.
CHAMP
Through the Public Health Service, CCHS is able to contract
with one local pharmacy in each office service area to provide lower prices for
certain medications. Choptank
Health’s Assistance with Medicine Program, or CHAMP, can help eligible
patients pay reduced prices for prescription drugs ordered by CCHS providers.
Who is eligible?
Any CCHS patient who has a family income within our sliding fee scale range and does not have
insurance or has insurance that does not cover prescription drugs.
Interested?
It is easy to apply by completing a sliding fee application
at any CCHS office. If you are
income eligible, you will receive a CHAMP card to use at a participating
pharmacy. You will still pay for
your prescriptions at the pharmacy, but your cost will be reduced.
Where can you use a CHAMP card?
Cambridge, MD – Craig’s Pharmacy
Hurlock, MD – The Drug Store
Denton, MD – Denton Pharmacy
Ridgely, MD – Ridgely Pharmacy
St. Michael’s, MD – Reeser’s Pharmacy
