School Based Wellness Centers

The School Based Wellness Centers administered by Choptank Community Health System, provide comprehensive primary medical, social, mental health, health education and dental services, designed to meet the psychosocial and physical needs of children and youth within the context of the school environment. Through a partnership with Caroline County Board of Education, the Caroline County Health Department and historically, the University of Maryland School of Nursing, CCHS established on-site Wellness Centers at 5 Caroline County schools; Greensboro Elementary, Federalsburg Elementary, Colonel Richardson Middle, Lockerman Middle and North Caroline High School.

In 2006, services will be expanded to both Caroline County high schools, North Caroline and Colonel Richardson High, as well as Ridgely Elementary School.

Medical services are provided by mid-level providers, with the support from CCHS medical center staff.  Providers in the schools have access to lab services and may provide prescriptions for acute problems.

The mental health services are provided through a contract between CCHS and Caroline County Mental Health Department, with licensed mental health providers located in 4 schools. These services will also expand to the additional schools during 2006.

 

The School Based Dental Program is an oral health prevention program that provides dental screenings, exams, and cleanings.  Based on the child's caries risk assessment, the program provides fluoride treatments, referrals for restorative dental care and dental sealants to children enrolled.  The intent of this program is to identify children with untreated dental disease and with no dental home and link them for definitive care.

Once the screening is performed by the dentist, a treatment plan is established for each child, with follow-up services provided by a Registered Dental Hygienist with the help of a dental support specialist.

During the 2004-05 school year, the SBDP expanded from the established schools with Wellness Centers to include Ridgely Elementary School, Denton Elementary School, Greensboro Head Start and the Judy Center programs.

There is no cost to the child or family for these in-school medical, dental and mental health services.  When possible, insurance billing will occur.

In addition to these services provided in the Caroline County Public School system, the School Based Dental Program is offered in two additional counties.  CCHS has partnered with the Talbot County Public School system to provide the mobile dental services to all elementary schools in Talbot County, as well as to St. Michael's and Easton Middle Schools.

In Dorchester County, CCHS has partnered with the Dorchester County Health Department to provide the mobile dental services at both Mace's Land Middle School and North Dorchester Middle School.  During the 2005-06 school year, expanded services will be offered at Sandy Hill Elementary School and the Judy Center in Cambridge.
