Choptank Community Health System provides medical and dental services in Caroline, Dorchester, Kent, Queen Anne’s, and Talbot counties, with a mission to provide access to exceptional, comprehensive, and integrated health care for all.
CCHS is a participating provider in Medicare, Medicaid, and most major insurance plans. A sliding fee scale is also available for any patient without insurance. As one of 1,400 nationwide Federally Qualified Health Centers and a Community Health Center, CCHS is able to use federal funding to expand current services and develop new programs to meet the healthcare needs in our area.
