Choptank Health Celebrates Expanded Care in Chestertown

Choptank Community Health System joined the Kent County Chamber of Commerce, community leaders, and partners on Sept. 22 for an open house and ribbon-cutting at its expanded Chestertown Health Center. The gathering highlighted a growing team and greater access to medical, dental, and behavioral health care for Kent County residents. 
Mobile Health Unit in Chestertown

Guests visit Choptank Health’s Mobile Health Unit outside the Chestertown Health Center during the Sept. 22 community open house. The event included tours, introductions to the care team, and a ribbon-cutting with the Kent County Chamber of Commerce.

Guests toured the health center at 126 Philosophers Terrace, met members of the care team, and learned how Choptank Health’s services work together to support children, adults, and families close to home. 

Guests also visited Choptank Health’s Mobile Health Unit, which brings well and sick care directly to Mid-Shore communities. Mobile services help reduce transportation and scheduling barriers while connecting patients with Choptank Health’s broader network of care. Same-day scheduling and walk-up appointments are available.

The Chestertown Health Center Provider Team

The Chestertown Health Center provides primary medical care for patients of all ages, including preventive and acute care, annual wellness visits, women’s health services, immunizations, screenings, chronic disease management, medication management, and patient education. The primary care team includes Caleb Consenstein, MD; Kristina Semonick, CRNP; Morgan Bleinberger, DNP, CRNP, FNP-C; and Lauren Burnett, DNP, CRNP, FNP-C.

The center also offers general and pediatric dental care. Pediatric dentist Beatrice Bautista, DMD, is welcoming new patients and expanding access to specialized dental care for children, including those with complex medical, developmental, behavioral, or special health care needs. 

General dentist Areeg Elmusrati, DDS, provides preventive and restorative dental care and brings advanced residency training in orofacial pain. Dental hygienist Lisa Clough, RDH, provides preventive care and oral health education as part of the Chestertown dental team. 

Behavioral health clinicians Carly Boughner, LCSW-C; Amy Piccaluga, LCPC; and Deborah Piez, LCPC, provide counseling and behavioral health support as part of Choptank Health’s integrated approach to care.
