School-Based Locations and Services

School-Based Health Centers on Maryland’s Mid-Shore

Choptank Health’s School-Based Health Centers provide medical, dental, nutrition, and behavioral health services for enrolled students and staff throughout Maryland’s Mid-Shore.

School-Based Health & Dental Centers are a partnership between Choptank Community Health and the County Public Schools. This program allows students to receive medical and dental care right at school. 
Care is coordinated with your student's primary care provider and/or primary dentist. Choptank Health will always obtain parent/guardian permission before seeing or treating your student.

 
School Based Health 
Choptank Health operates School-Based Health Centers in Caroline, Kent, Queen Anne's, and Talbot counties, with dental services provided in Dorchester County. Any student enrolled in the SBHC program can receive care at any location and can receive care regardless of online or in-person attendance. 

Each Choptank Community Health School-Based Health Center is staffed by licensed, advanced practice clinicians, including Certified Nurse Practitioners and Physician Assistants.

The Centers function like a typical doctor’s office, with medical providers assessing and diagnosing illness, writing prescriptions, performing lab tests—including strep and COVID—and performing physicals, including for school sports.

Dental services including dental screenings, cleanings, sealants, fluoride treatment, and emergency visits—are also offered in Dorchester, Kent, and Queen Anne’s counties, as well as in Choptank Health’s Caroline and Talbot County’s School-Based Health Centers, with care coordinated with your child’s primary dentist.
