As we begin Choptank Health’s 46th year, we are proud to stand alongside a national movement celebrating 60 years of Community Health Centers—a bold experiment in health equity that continues to transform lives, as shown in this video.What began with a promise to bring care to America’s most overlooked rural and urban communities is the same promise Choptank Health lives out every day on Maryland’s Mid-Shore.
Inspired by a mission rooted in dignity, access, and whole-person care, we are honored to serve more than 34,000 Mid-Shore individuals and families with medical, dental, and behavioral health services. And we’re proud to extend our healthcare reach into the most rural areas through telehealth, mobile health, and School-Based Health programs.
Like community health centers nationwide, Choptank Health reaches beyond the exam room to reduce barriers, address social drivers of health, and ensure no one is turned away.
